Appointment questionnaires

We may ask you to send us some photos of the issue you are having. By sending us photos you consent to them being saved in your medical record and reviewed. For tips on how to take a photo, please click here. 

PHQ and GAD scores are questionnaires which ask a few questions about your mental health. The scores help us to offer you the most suitable support in an appropriate time scale. These questionnaires are available via SMS link or in the practice on paper

ACT scores are a set of questions which patients may be asked to complete in relation to their asthma. Not all patients require a full asthma review each year and our asthma nurses use this information to ensure patients are offered an appointment when they need one.
