Sick notes

To request a sick note:

• use Animaima to request a sick note
• phone or visit the surgery if you are not able to use the online contact tool

For extensions to sick notes please use the contact methods above. We are not able to issue sick notes until the day your current note runs out, and we do not issue fit notes. 

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
